Theadmissions specialistcoordinates hospice care admissions, assesses appropriateness of potential hospice admissions, provides case management and discharge planning assistance from inpatient settings. The individual acts as liaison to community healthcare professionals offering education and consultation on hospice care; generates referrals to the organization. Maintains communication with non-admitted/ discharged patients. Centerville location * Part-time (20 hr/wk) Admissions (Des Moines location) * (1) Weekend Package Essential Duties: * Receives, monitors, and evaluates potential admissions for the organization. Performs initial case management with third party payers; educates the patient and primary care giver about the hospice benefit. * Obtains informed consent for care and initiates admission process. Provides documentation of direct patient services. Communicates with team director for assignment of staff. * Provides hospice services information to patients, families and the community. Participates in education for contract facilities/services. * Coordinates discharge planning for hospice patients who are admitted to the hospital. Serves as a liaison to hospice staff to ensure a smooth transition. * Maintains on-going communication with hospital discharge planners and social workers. Provides follow-up communication to attending physicians regarding non-admissions. Maintains communication with non-admissions to provide follow-up education and on-going assessment for hospice qualification and appropriateness. * Markets the organization through scheduled visits by establishing professional and public relationships with physicians, nursing homes and other health care entities to encourage and develop current and potential referral sources. * Monitors customer satisfaction and preferences. * Participates in quality assessment/performance improvement planning and process. Requirements: * Registered nurse required; bachelor's degree preferred. * Two years hospice or related experience. * Marketing experience preferred. * Ability to plan, organize and utilize teaching principles in working with staff, patients and primary care givers. * Ability to exercise independent judgment; strong communication skills. * Ability to create and maintain professional relationships. * Strong computer skills with the ability to learn work related software.
