Acute Care Practitioner Department: Psychiatry Staff Schedule: Full time Shift: Day Shift Hours: M-F 8a-5p must be flexible for some evening hours Minimum Hourly Rate: Commensurate with Experience Job Details: * General Summary: The Acute Care Practitioner performs case management core functions which include: the screening and identification of patients: Establishment of an effective relationship with patient/family; Assessmentof patient/family strengths and needs; Obtainingcomplete health, social, environmental and developmental history; Perform physical exam and developmental evaluations; Development of a family-centered plan of care; Procurement and coordination of services; Monitoring and evaluation of progress. Minimum Requirements: MS in Nursing with current Ohio licensure; satisfactory completion of a PNP or FNP program; Certificate of Authority to practice as an Advanced Practice Nurse in the State of Ohio with Prescriptive Authority. Successful candidate will have good assessment and critical thinking skills. Must possess excellent leadership, interpersonal skills with an advanced level of critical thinking and decision making abilities. 1-2 years pediatric experience in a Behavioral Health setting is required. Strong customer service orientation, enjoy working in a fast paced and dynamic environment with ability to function in a multi-disciplinary team. Minimum Physical Requirements:
1. Must be able to see, hear, stand, walk, speak, read and perform manual tasks with or without accommodation, and care for oneself with little or no difficulty.
2. Must be able to independently lift up to 25lbs.
3 Must be able to move or reposition patients of any weight or size with the assistance of another person(s) and /or equipment.
